How do I change my personal details?

Please inform the Receptionist if you change your name, address or telephone number. We must have your telephone number even if it is ex-directory so we can contact you in an emergency. It will be kept strictly confidential.

How do I book a home visit?

Home visits are made for those patients who, because of their condition, cannot get to the surgery. If you would like to discuss whether a visit is necessary, please telephone the reception staff before 10:30 am on 0191 543 8807.

How do I book appointments online?

Please contact the reception for a user name and password. A form of identification will be required.

Why is there a queue at reception?

If you are checking in for an appointment, please use our designated check in screens. We like to make sure each patient has the time that they need with the receptionist and in a confidential manner.

Do I have to take my prescription to a pharmacy?

You can send someone to collect a prescription on you behalf if they know some of your personal details however they will need consent from yourself and identification must be shown. A pharmacy of your choice may also collect for you, please speak to your local pharmacy to arrange this service.
