New Patients

Alliance Medical Group operates an open list policy.

It is quick and easy to register and change doctors and takes only a few minutes to organise. If you want to register at the practice, please inform our reception staff. Our team will provide you with the correct documentation.  You can also use our website to register at a time that suits you.

We ask all new patients to attend a new patient check with our healthcare assistant. This gives us an opportunity to meet you and your family. It also allows you to find out more about the practice. All patients must read our practice policy. This is to ensure that patients know what services we offer. It also helps us to meet your expectations and health needs.

Registering digitally

Did you know that you can now register as a new patient at one of our practices online? All you need to do is select a practice from the options below and click the link to register:

New Patients – Require a new patient health check with a Healthcare Assistant. This is a 30 minute appointment.

Change of details – We aim to ensure all patient identifiable data is up-to-date. If you change your name, address and home or mobile telephone number, let us know as soon as possible.

Summary Care Record – All patients are signed up to Summary Care Record. This allows our health partners to access your records. If you don’t want to allow our partners to access your records, contact the practice to “Opt Out” of this service.

Online Services – You can register for online services at any time. Please complete the access form here. This allows you to a) make an appointment.  b) request repeat prescriptions.

Violence and Aggression – We operate a zero-tolerance policy towards violence, aggression or verbal abuse. Any patient who behaves in such a manner towards a member of our practice staff or another patient of our practice, on or off surgery premises, will be removed from our patient list and if necessary, reported to the Police.

Please let us know if you change any of your details.
